Home and Community Re-Entry
NRC Feldman and Associates Inc. helps individuals with an acquired brain injury and other neurological impairments return to their home and community. We work with individuals and families to develop compensatory strategies, and to modify their environment so that they can fulfill their roles in meaningful ways while at the same time maintaining and enhancing their quality of life.
Our home and community re-entry model incorporates the diverse and complementary skills of occupational therapists, speech-language pathologists, vocational counsellors, social workers, physicians, physiotherapists, psychologists, rehabilitation support workers, community service providers and clients.
Interventions:
- ADL/ANL assessment and training.
- Attendant care assessments.
- Cognitive and communication assessments and therapies.
- Ergonomic assessment, training and modifications.
- Home safety assessment, training and modifications.
- House keeping / home making assessments.
- Individual and family psychosocial assessment and counselling.
- Recreation and leisure training.
- Sex and sexuality education.
- Supervision of personal support workers/attendants, and rehabilitation support workers.
